The Los Angeles Dodgers have agreed on a trade that will send Matt Kemp to the San Diego Padres, a Padres official confirmed to ESPN.com.

Los Angeles will receive catcher Yasmani Grandal, pitcher Joe Wieland and another prospect, the source said, while the Padres get Kemp and catcher Tim Federowicz.

The prospect is right-hander Zach Eflin, according to multiple media reports.

The Dodgers will also send about $32 million to San Diego to cover part of Kemp's remaining $107 million salary, a source told ESPN.

The trade helps alleviate a logjam of outfielders in the organization. Andre Ethier and prospect Joc Pederson will vie for playing time, with Yasiel Puig holding the starting job in right field, and Carl Crawford in left. The Dodgers could also use role player Scott Van Slyke in the rotation.

Kemp hit .287 last season with 25 home runs and 89 RBIs. The 30-year-old, who missed 145 games with multiple injuries between the 2012 and 2013 seasons, finished second for the MVP award in 2011 after collecting 39 homers, 126 RBIs and 40 stolen bases.

The agreement is the latest in a flurry of moves for the Dodgers, who, in a busy Wednesday, acquired second baseman Howie Kendrick, infielder Enrique Hernandez, reliever Chris Hatcher and minor leaguer Austin Barnes. Los Angeles also added veteran shortstop Jimmy Rollins and agreed to a four-year deal with pitcher Brandon McCarthy, sources told ESPN.

Since taking over on Oct. 14, president of baseball operations Andrew Friedman and his general manager Farhan Zaidi have made 10 different trades (three Wednesday alone), claimed two players off waivers and have signed one free agent (McCarthy), according to ESPN Stats & Information.

LOS ANGELES -- The San Diego Padres were reviewing medical information before signing off on a trade for Dodgers outfielder Matt Kemp, a source said, with a Thursday deadline to complete the deal fast approaching.

Should San Diego pull out of the trade -- according to USA Today, Kemp's physical revealed arthritis in both hips -- two other trades and three other teams could be affected.

The Padres and Dodgers agreed to a trade one week ago that would send Kemp, catcher Tim Federowicz and $32 million to San Diego for catcher Yasmani Grandal and pitching prospects Joe Wieland and Zach Eflin. The Dodgers then planned to trade Eflin and their pitching prospect Tom Windle to the Philadelphia Phillies for shortstop Jimmy Rollins.

In addition, if the Kemp deal crumbles, the Padres might have to rework a three-team trade with the Washington Nationals and Tampa Bay Rays agreed to Wednesday since their interest in catcher Ryan Hanigan -- one of the pieces in that trade -- presumably would be affected by whether Grandal remains a Padre.

The Dodgers are upset that Kemp's medical information leaked publicly, sources told ESPN.com's Ramona Shelburne.

If San Diego general manager A.J. Preller was trying to pry better terms out of the Dodgers, such as more money to cover Kemp's remaining $107 million in contract obligations, the leak of Kemp's medical information could give the Dodgers room for a grievance. The 1996 Health Insurance Portability and Accountability Act protects the privacy of patients' medical information.

Kemp was the runner-up for the National League MVP in 2011, but he has dealt with a long string of injuries since April 2012. He underwent two shoulder surgeries and a major operation on his right ankle and dealt with three hamstring strains. Until now, he had never been diagnosed with a hip ailment.

The 30-year-old hit 17 of his 25 home runs and drove in 54 runs after the All-Star break last season. The Dodgers have been looking to trade an expensive veteran outfielder -- either Kemp, Andre Ethier or Carl Crawford -- for more than a year.

SAN DIEGO -- Slugger Matt Kemp has joined the Padres after San Diego and the Los Angeles Dodgers finalized a five-player trade Thursday night.

The teams agreed to the trade last week during the winter meetings in San Diego. It was the first of three big deals by new Padres general manager A.J. Preller, who also has added outfielder Wil Myers, the 2013 AL Rookie of the Year, from Tampa Bay and All-Star catcher Derek Norris from Oakland.

The Padres, desperate to pump up the worst offense in the majors, also get $32 million from the Dodgers to help offset the $107 million remaining on Kemp's contract. The Padres' obligation of $75 million to Kemp over five years becomes the biggest deal in club history.

Kemp twice made the All-Star team during his time in L.A., as well as winning two Gold Glove and two Silver Slugger awards. His 182 homers rank fourth in Los Angeles Dodgers history.

San Diego also gets catcher Tim Federowicz. The Dodgers receive catcher Yasmani Grandal and right-handers Joe Wieland and Zach Eflin.

A.J. and his team did their due diligence, conferred with our medical team and ultimately were comfortable moving forward."

The deal was completed despite some possible lingering ill will between the Dodgers' front office and Preller because of the belief that Kemp's physical results were leaked to help San Diego pry further concessions out of its NL West foes, sources told ESPN.com's Ramona Shelburne. The Padres did ask for additional compensation from the Dodgers after Kemp's physical, but Los Angeles refused.

The deal was cast into some doubt after USA Today reported Thursday that Kemp's physical with the Padres revealed arthritis in both hips. The Dodgers were upset that Kemp's medical information leaked publicly, sources said, though one Dodgers source told ESPNLosAngeles.com's Mark Saxon that the team never felt the deal was seriously jeopardized.

Former Dodgers scouting director Logan White, who now works in San Diego, was viewed as one of the driving forces behind the deal's completion.

Kemp has had shoulder and ankle operations in recent years. Last May, he was groomed to play left field because the Dodgers weren't confident he could handle the defensive demands of playing center field after undergoing ankle surgery the previous offseason.

The Padres had a historically awful offense in the first half of 2014 before pulling out of it to finish third in the NL West. Still, they were at the bottom of the majors with a .226 average, .292 on-base percentage and 535 runs. Their 109 homers were the second-fewest in the bigs.

The 30-year-old Kemp had a strong second half in 2014 for the NL West champion Dodgers. He finished the season with a .287 average, 25 home runs and 89 RBIs, hitting 17 of those homers after the All-Star break while driving in 54 runs. He led the major leagues in slugging percentage during that span.
