what size top loaders do I get for jersey and patch cards, seems to be several different choices

That depends on the thickness of the card.

Nope. Measure the card yourself, there's a wiki that gives the conversion into PT. or MM. PT = points and MM = millimeters. I always just buy a few packs of different sizes and go from there.
